Requirements

  • Letter of Good Standing
  • Background Check 
  • Liability Insurance (we can provide)
  • Copy of UMSLE Step 1 (passed)
  • Proof of immunity to Mumps, Rubella, Measles, and Varicella
  • Proof of Hepatitis B vaccination or proof of positive antibody titer
  • Proof of PPD within the last year or chest x-ray in 1 year if PPD Positive
  • HIPPA and OSHA Training
  • Physician physical exam within the past 6 months
  • Must be an American citizen or have a valid Visa (visa letter can be provided)
  • Identification Card (Driver’s license or state id or passport)
  • Negative Drug Screen (conducted at the hospital for $20)
  • Valid CPR card
  • CV or Resume
  • Flu Vaccination (seasonal)
  • Covid-19 Vaccination (first 2)
